영문 법인인감카드발급신청서(견본)
영문 법인인감카드 발급신청서에는 제출 회사의 상호·등기번호·본점, 신청인의 직위·성명·주민등록번호를 기재하고, 발급 사유는 최초 발급·분실·훼손·인감증명서 발급용 등에서 선택합니다.

1. The PIN number to be used for the (re)issued seal card shall be entered in the above PIN number section, which shall be 6 digits in arabic numerals. The PIN number is very important when using the seal card to apply for the issuance of a seal certificate, therefore precautions are required not to disclose it to any unauthorized party.
2. When applying for reissuance of seal card, you must pay the fee as prescribed in the Registration Certificate and Commission Fee Rule. However, this is not necessary when returning the seal card.
